What do we mean by interdisciplinary care?

While a multidisciplinary team brings together different professionals, each tends to work more independently. An interdisciplinary team goes one step further: all members are in regular communication, sharing insights and combining their expertise into a single, coordinated rehabilitation plan.

At Askham Rehab, this means our physiotherapists, occupational therapists, speech and language therapists, neuropsychologists, nurses, and support workers work alongside our Consultant in Neurorehabilitation Medicine, Dr Fahim Anwar, to ensure that every decision is made with the resident’s unique goals in mind.

How we put this into practice

Collaboration at Askham Rehab is part of our daily routine.

  • Weekly team meetings allow us to review progress and set new goals together

  • Shared documentation ensures consistency across disciplines, avoiding duplication and reinforcing a unified plan of action

  • Family involvement is central. We actively seek input from residents and their loved ones, recognising that recovery is most effective when it reflects personal aspirations as well as clinical needs
